We are developing our new 5-year funding proposal (2018-2023) and strategic plan to be submitted to the Administration for Community Living (ACL) in March of 2018. We are asking for feedback from CDS Faculty, staff, students, stakeholders, and individuals in the community (with and without disabilities). The data collected from this survey will be used to make sure our community research and program implementation efforts address the needs of our community. Data will not be used in any publications.
