GHP Services | GHP Services for DD/ID Waiver Participants
Participants and family members work with a transition coordinator to design a package of services that meets their specific needs and helps the participant live safely and successfully in the community. Participants can also choose the waiver providers with whom they wish to work. Services may include:

- Transition Coordination
