About Us
The Going Home Plus (GHP) project is funded by the Centers for Medicare and Medicaid Services Money Follows the Person Rebalancing Demonstration grant through 2012. Hawaii’s GHP project is a collaborative effort between the Department of Human Services (the state Medicaid agency) and its community partners. The Center on Disability Studies at the University of Hawai‘i is a GHP project partner and will also conduct the GHP project evaluation.
