The U.S. Department of Housing and Urban Development (HUD) today joined President Obama’s commemoration of the 10th anniversary of the Supreme Court’s Olmstead decision by announcing rental assistance to 4,000 non-elderly families with disabilities, including 1,000 vouchers specifically targeted to those transitioning out of nursing homes and other facilities.

Project Overview
The purpose of this project is to transition individuals from hospitals, nursing facilities and ICF-MRs to community settings. The project will review and consider individuals currently institutionalized, including the elderly, medically-fragile children, and youth and adults with disabilities who express a choice of community living.
This five year project, beginning in 2007, is funded by a Centers for Medicare and Medicaid Services Money Follows the Person Rebalancing Demonstration grant. The project is a collaborative effort between the Department of Human Services (the state Medicaid agency) and its community partners. The community partners will serve as members of a Stakeholder group to plan the successful implementation of the project. The Center on Disability Studies at the University of Hawai‘i will be a partner in the pre-implementation phase and in the evaluation of the project.
To be eligible for Going Home Plus, you must be:
- Receiving Medicaid
- Currently living in a hospital, nursing facility, and/or ICF-MR for at least 6 continuous months
- Nursing facility level of care
